Schedule

9:30 a.m. – Check in at Granny White Park. Follow registration signs. Late registrations accepted at this time. Pick up T-shirts and submit collected donations.

10:00 a.m. – Walk begins and is approximately one mile. Strollers or wagons are welcomed, but bikes and rollerblades are not allowed. Water will be available for all.

11:30 a.m. – 1:30 p.m. – Enjoy family fun and activities. Food will be provided by Chick-fil-a. We will also have a silent auction. Please bring a gift to contribute to the auction.

Registration

Deadline Sept. 17, 2004

Registration before the day of the Walk:

$15 per adult; $5.00 per child under 12

Registration on the day of the Walk:

$18 per adult; $7.00 per child under 12

Each person registered by the September 17 deadline is guaranteed a T-shirt celebrating this year’s event. Note that registration fees are not considered tax-deductible donations. Mail this form with registration fees and all collected donations to:

In consideration of your acceptance of this registration, I hereby for myself, spouse, family members and/or anyone included in this registration release any and all rights and claim for damages against CHI and CHI Foundation and their directors, officers, members and representatives. A parent or guardian must sign if participant is under 18 years of age.

Donations may be sent with your registration and/or made at the time of the Walk.
