Expedited Procedure for birth certificates (which includes state authentication)
You must use an express mail service (such as Fed-ex or equivalent). Use a 9x12 envelope addressed to:

Place the following items in this 9x12 envelope:
Request letter – In the letter of request state that you are requesting an exemplified copy with apostille seal of your birth certificate for an international adoption. Your letter must include the full name as it appears on the birth certificate; the date of birth; the town, city, or village where the birth occurred; the father's name; the mother's first and maiden name; and your relationship to the person named on the birth certificate. The fee is $15.00 per copy. Checks or money orders should be made payable to the New York State Department of Health. DO NOT SEND CASH. You must also be sure to specify the country for which the apostille is needed.

Getting apostilled copies for birth, marriage, and divorce certificates from New York City, not New York State, is a several step process, and each office says that they will not forward documents after they process it.
Step 1. Contact the Bureau of Vital Records in New York City and asked for the long form of your birth certificate along with a letter of exemplification copy of the document.
