Form was including in a prior package sent to you.

signed Seeking Confirmation Letter
If you identified a waiting child, you will not have this
form.

your Travel Approval (Notice of Coming to China)
The following
will be emailed or faxed:

Inside
China Travel Summary of Fees from CHI:
Fee will include all inside China travel arrangements and
the orphanage donation of $3000, if CHI can wire the
donation to your child's orphanage. We ask that fees be
paid to CHI in the form of a certified or bank check.

Detailed Inside China Travel Itinerary:
Including flight numbers inside China, phone number of your
coordinator and contact information for staff in Beijing
office and all hotel telephone numbers.
